On 3/7/79, a flight of six F-4Ds from the 4th Tactical Squadron, 388 Tactical Fighter Wing from Hill AFB, Utah, departed Nellis AFB, Nevada at 1:12pm as part of a Red Flag 79-4 interdiction mission. Call signs Pall 21 through 26 joined up following single ship take-offs for the flight to the Red Flag air refueling track in Caliente Military Operating Area. The mission was to ingress to the target area as two ship elements after refueling. Pall 21 and 22 (Pall 22 was the mishap aircraft) began their descent immediately after departing the air refueling track to start their low level ingress to the target. The weather was clear with 30 miles visibility. During the low level flight, the aircraft impacted the ground destroying the aircraft scattering wreckage for 2,700 feet and fatally injuring the pilot Robert F. Johnson and navigator Jay F. Hagen.
